Procedure to Change my Surname

Hello, My name is Vikas U M. While applying for passport 10 years back, i have given my name as Vikas Ujjani Mata (expended my initials). Now i want to correct my Surname to Vikas Ujjanimath. Please suggest me the legal way to getting this done.

You will have to make an affidavit and get the same notarized or signed by your local SDM. The affidavit must mention the current surname and desired surname and briefly mention the reason for change of surname. Once the surname notarized or signed by your local SDM, it comes in force immediately.


Thereafter you are required to publish your change of surname in a newspaper one official language and one in English.



This is required as per law, so if anyone has objection they can take it up with authority.

In case you wish to change your surname in passport then please apply to the nearest Passport Seva Kendra (PSK) on Fom II (Misc Services) which is available at all PSKs and also on line. This will involve reissue of a new passport.
