Procedure for correction of name in birth certificate

Date of birth of a daily wage worker in PWD was changed in Panchayat record from 1950 to 15-07-1957 by secretary of concerned panchayat in 1995. Affidavit of father of daily wage worker is the basis of correction in D.O.B.as the worker was totally illetrate. In 1995 the daily wage worker was regularised in service and she presented certificate of concerned Panchayat with her changed D.O.B. as 15-07-1957 for maintaining her personal official service book and she is due for retirement 0on 31st July 2017. It is not clear from official available that on what basis secretary of concerned Panchayat admitted the request for change in D.O.B.Please guide me to know the mistake done by secretary of concerned panchayat and also suggest the accurate procedure of making change in the existing D.O.B. of any indivudual/govt. employee.

If a person decides to change his or her name for any reason, it will require providing a few important documents and giving the reason behind the change. Sometimes mistakes happen.

If the child’s birthplace, sex, or other information is not correct on the birth certificate, it will be necessary to get changes made to reflect the correct information.

This will often require some form of proof in order to validate the change also have to obtain an affidavit from the court and submit the same under a respective section applicable under a law in your state to the municipal committee.
