Mother's name for child's educational document

My question is one of my friends wife named expired in 2004 leaving behind one girl child of 4 yrs.Then he remarried in year 2006 to women named. Now the girl child anagha is now to appear in board exams in which mothers name is to be filled now which name she should fill whether of first wife or second.What is the correct thing to do?

File in second wife name as she is legally alive mother of his daughter.
You have to show your second wife as her mother so that in future if any question arise your second wife can always make appearance and sign documents

Has the second wife legally adopted the child? The second wife is the step mother and her name can be entered as 1st wife is no more , hence no one to challenge it .As in this case the biological mother is no more ,the 2nd mother's name can be filled.

You have to mention the same mother's name that will be appearing on her Birth Certificate. Mother's name don't change just on the basis that you got re-married due to death of your earlier wife or for any other reason.
