How to transfer grandfathers name on 7/12 paper if he is actual owner

A land was purchased and registered on my grandfather name in 1930 but forgot to transfer the name on 7/12 paper. And now in 2017 when i saw 7/12 paper the name are of different people who never owned that land. Now how i can transfer the name of my grandfather on 7/12 paper. And also on that land i have built a house which is on my name. Please guide me.
Please submit the application for mutation plot in your name along with necessary documents like copy of sale deed, tax receipt etc. If they are not doing it in one or two months' time, you can follow up with RTI Application - seek the note sheet and orders of competent authority on your application etc.
