How to tackle discrepancies in name in various certificates?

My name is stated as Madhav kumar patel in my dob certificate 10th and 12th, and in bank, pan, college degree,passport it is Madhav Patel.Will affidavit of name of same person is sufficient?
Whether it would be problematic or not would depend from one government department to another. You may enquire about the application process through an RTI if need be. Further, you may make an application in writing to the board to have a duplicate/corrected certificate or marksheet issued if required.
