How to remove ex spouse name from passport

I am divorced Muslims.I got divorce before 4 months.my question is in my passport my ex spouse name is available.how I remove his name from passport.
You will have to Apply for Re-issue of passport with the selection of the required change of particulars and show the certified copy of divorce decree to passport authority to remove the name of your ex-husband and re-issue of the passport.
