How to correct father's name in birth certificate of his child

I have lost (misplaced) My daughter and wide discharge summary issued.Now i need it to attach it in MCD office for correction of my name in my baby's birth certificate. i have visit hospital to request issue discharge summary as on records on your hospital but hospital management refuse my humble request what should I do?

It can be changed by an appropriate application to the concerned municipal corporation addressed to the registrar of births and deaths. The application shall be accompanied by appropriate enclosures supporting your claim of change in details. Apart from this you need to do certain things such as joint affidavit of parents on 100 rupee stamp paper attested by class I magistrate, copies of change in details advertisement published in two local newspapers (depending on the municipal corporation), photo ID of parents and educational documents of father. It is advisable to contact a lawyer for the affidavit and the application

you just name the hospital they will take the data directly from the hospital if it is necessary for them and if you need to take the copy of discharge summary you can ask any clerk of the hospital if they refuse to give you the same you can produce the bill given by them to you in lieu of the discharge summary to the mcd they will register the name according to the aadhar card of yours.

You have not mentioned why the hospital Management has refused to issue the copy of discharge summary to you ?
The copy of these same must have been filed in the MCD when the Birth Certificate was issued and hence you can take a copy from MCD also by filing an RTI. Further you can apply for correction of your name by submitting proof of your correct name and spelling, If there is any spelling error. Hospital also keep record of all births and forms submitted to MCD and can give on their letterhead by certifying that there were mistake / error in the form submitted to MCD after birth.
