How to change name for application in passport and pan card

Sir my name in all certificates is MULLA AHMAD BASHA ..in which MULLA is my surname and except all is my name..so can you please give me the idea of FIRST NAME LAST NAME MIDDLE NAME in accordance to my name. i am confusing because of in all my i cards my name is as mentioned earlier , but in filling the PAN or PASSPORT they are asking first middle last names..basically iam from ANDHRA PRADESH , we dont have any fathers name incuding in our names
