First issue a notice through your Advocate to concerned dept to correct the name by producing the records to show the correct name
Then file a suit for declaration as may be advised by ur advocate
What are the documentary evidence you have in your custody is not mentioned in ur question
Any way show those documents to ur advocate at the time of issuing notice
It is a statutory notice as required under section 80 of civil procedure code
After issue of notice wiat for statutory period ad per law then file the suit

Generally SSLC Board in Karnataka prefer Court Order declaring such a name change. Mere affidavit signed by a Notary may not be helping the cause. Suggest you to approach the said Board and ask them what legal measures you need to take. Accordingly Advocates can assist you suitably. By experience, I can say that you may be most likely asked to get the Order from the jurisdictional Civil Court for such a name change or to correct the name.
