Court order required for issuance of birth certificate

Hi, my daughter was born in Oct 2012 in a private city hospital in bangalore, after her birth the hospital staff gave me a form to fill up for birth certificate and submit it back within 10 days. The form had no letter head, just few items such as father, mother and child name, date of birth, address, etc. Due to my negligence I forgot to give back the filled up form to the hospital. Later I applied for birth certificate with the BBMP office and they told me that the certificate cannot be issued as there is no record and the birth is not registered with them. How do I proceed in this case?

please go to hospital where your daughter was born with proper application cum letter which should disclose Name of father, mother, date, bill date and amount paid, Doctor who was involved in your delivery. Ask them to issue fresh letter and then approach BBMP office (special Dept) ask for form, You may have to submit that with Affidavit so please consult Lawyer who will make affidavit and who may meet officer concerned on your behalf.
