Correction of my and my father name in 12 th certificate

I am munigoutam kumar.i passed 12th exam in 2016 . actually sir,I wrongly wrote my name Kumar munigoutam in 12th certificate.give me suggestions
U can change yr in correct name , consulting yr council and giving same evidence of name.
Y will have to file an application for the same supporting with Court affidavit and adhar and voter id as evidence
