Change of father's name in educational certificate

My uncle (amit verma) look after me because at the time of my birth ( 10-07-1982) my mother had died. He written his name as father in my education documents. How i replace the name of my uncle (amit verma) with the my original father ( neeraj goel). My father (neeraj goel) also shown my name as a son in his ration card.
