Annexure D wife name change to new name after marriage -passport

Is notarisation of Annexure D-(a document mentioned by passport office meant for changing name of wife to new name after marriage )valid if done in banglaore but where the application will be made at gurgaon passport seva kendra . Also if parents are not new married ,they are married for more than 40 yrs ,and all documents of mother already show her name after marriage ,including her husbands name ,does the Annexure D still need to be filled?
