Want to remove ex husband's name from child's birth certificate

I have been divorced from my ex husbands and my 3 years baby is with me and it is declared that in future I can't demand anything for my baby. Now I want to remove my ex husbands name from my child's birth certificate and want only my name to be there. how to do it?
According to the rules birth certificate can be issued in sole name of biological mother.for that go to nagar Nigam and file application along with certified copy of divorce decree to issue certificate with mothers name only.

As per ur statement u r interested, to removed ur ex husband name from birth certificate ,first u apply before appropriate competent authority five removal of ur husband name ,if authority refused then moved to appropriate court for correction name in birth certificate. Rest on VI's a vise
