Hello, An AOR(Advocate on Record) can file the CAVEAT in Supreme Court and in that case, there are some specific formalities and format is available. But a common person cannot file it directly. Only CAVEAT filing charges are very nominal.

See a Caveat petition is a precautionary measure which is undertaken by people usually when they are having very strong apprehension that some case is going to be filed in the court regarding their interest in any manner. The caveat petition remains in force only for 90 days and if during that duration no case gets filed from the opposite side than you have to again file a fresh caveat petition as new in the court.
The format will be of no help to you as the petition will have to be filed by an Advocate. So, do consult with relevant reasons and documents of the case in which you are apprehending an appeal/special leave in Hon'ble Supreme Court.
