Surname change of girl after intercaste marriage

I have done intercaste marriage, after it i am confused regarding my surname change. Because my phd is ongoing, my job, my salary account, each having my first surname, my main question is that after suname change which type of documents required and facing the problems

you have to make an affidavit for change your name then after you have to file an application to sdm for change your name then you can application for publication to gazat and then you can show your name
