Signing an undertaking form on backdate

My college authorities have taken my blood sample and urine sample without undertaking form. They want me to sign the undertaking form with a back date. Undertaking form is where they can take legal actions against me. Please help me withis small query.

It depends on why the blood and urine sample was collected in the first place. My advise would be to not pre-date and sign any undertaking. However if you are of the opinion that signing the undertaking form is not going to affect you in any manner, then you may go ahead and sign it. However further information is necessary .
