Need first class magistrate in Goregaon East Mumbai

I lost the duplicate copy of lic which I need to provide also though I found the original and submitted the same to lic along with my KYC as I need to surrender the lic due to an emergency so i need an affidavit from first class magistrate stating that I misplaced my duplicate copy
