ISSUE A GAP CERTIFICATE/ AFFIDAVIT

I dropped for NEET 3 times ( 2019 - 2021 ). Now, I want to take admission in college so I want a gap certificate. I want to know that can I apply for it online ? If yes, what's the official procedure.
