Affidavit required for duplicate PAN card cancellation

There are two PAN cards that have been allotted under my name and I would like to cancel one and retain the other. I had reached out to the income tax office and they requested for an affidavit regarding the same.

1. you have to get it done by the advocate to submit the affidavit done and produce it before the authority. 2. you have to specify the reasons and the consequences and to mention each and every reason in the affidavit properly.
