What is the procedure to transfer ownership of property

Me and my wife are owners of a flat on which bank loan is going on. EMI are paid by me. Now due to some difference, i have to transfer the flat to my wife name and bank loan also to her. Please advice the way ahead and the procedure?

First at same time you and your wife both owner of flat which is under lone it's not possible?
You can transfer you flat in the name of wife by way of release of old right registered deed hand over all rights of property to your wife
