Can annexure-D of passport be used as valid marriage proof

I need to ask that Annexure-D of passport can be used as a valid legal marriage proof or act as a marriage certificate for all kinds of documentation required in various govt. and other institutions. Please, specify in detail the comparison between both if exists.
No it cannot be used as a valid marriage certificate. Annexure D of the passport form says that it's a Specimen Affidavit to be submitted along with application for passport by a woman applicant for change of Name after marriage.
