What shall be done when one person has two different names

Reference to above subject, prior to marriage my wife was known by name XYZ, after marriage she was given new name ABC, now some of documents like PAN CARD, bank account and property registration papers, her certificates and nomination in insurance are in name of XYZ, but aadhar card , voter card, PASSPORT and birth certificates of my children carry name ABC. what shall be done?
