Regarding change of surname what to do

Sir..so my surname jadhav is mistakely recorded in my academic...but my parents surname is Chavan..so is it necessary to change surname from jadhav to Chavan or is there any better way
Hi, please get an affidavit ready for the change of name and mention the correct surname in the affidavit and notarize it through an advocate. Submit the same in the magistrate court for name change and get an order passed. This also should be published in an daily newspaper publication and that would be sufficient to get the surname changed in all the academic records and if not changed not to worry as the order is with you as proof of the correct surname as passed by the Hon'ble Court.
