Is filing of form 13 & vakalatnama mandatory along with 3rd party affidavit, if a lawyer files for copying application on behalf of respondent.

A lawyer cannot act on behalf of a party, including applying for copies of the case record, without a valid Vakalatnama (Authorization) duly signed by the respondent and accepted by the advocate. If the lawyer is already on record for the case, the existing Vakalatnama suffices.
