Now they asking for her death certificate. But her certf. mentions her pet name not her official name. Need to get the 1994 death certificate corrected. I'm sorry for your loss. As per Haryana rules, for correction in a death certificate (like changing the name from pet name to official name), you need to apply to the Registrar (Births & Deaths) of the concerned Municipal Committee or Panchayat where the death was registered. Since the death occurred in 1994, this is an old record correction, which requires supporting documents such as the voter card (with the correct name), an affidavit, and possibly a newspaper publication. You will also need to file an application under Section 15 of the Registration of Births and Deaths Act, 1969 along with a declaration from a Magistrate. The Sub-Divisional Magistrate (SDM) or Tehsildar in your area usually has the authority to approve such corrections. You should contact the Civil Registrar Office (MC/Gram Panchayat) where the death was originally registered, and also visit your local SDM office to understand the affidavit format and process. Once corrected, you can resubmit the updated certificate to the pension office.

Visit the Registrar of Births and Deaths (in the area where the death was registered in 1994 — Municipal Corporation/Panchayat Office). Submit: Original 1994 death certificate (with the wrong name). Voter ID or any document showing her official name. A notarized affidavit stating both names belong to the same person. Any other supporting proof (marriage certificate, ration card, etc.). File a written application for name correction under Section 15 of the Registration of Births and Deaths Act, 1969. Some offices may ask for a newspaper ad declaring the name correction. Pay the required fee and follow up regularly. Once corrected, submit the revised death certificate to the pension office to process your mother's pension.
