Sworn affidavit before first class magistrate

19-Nov-2023 (In Civil Law)
Hey I've been asked by NIOS to submit a sworn affidavit before a first class magistrate as my father's name was entered wrong and that is needed to correct. Please help me this is their statement: sworn affidavit on a non-judicial stamp paper of Rs.10 making necessary declaration with respect to the request made for necessary corrections indemnifying the NIOS from any claim against such corrections by the third party and it should be notarized before the judicial First Class Magistrate
