To obtain a Guardian Certificate for your 54-year-old brother suffering from Parkinson’s disease, you must apply through the Local Level Committee (LLC) under the National Trust Act, 1999, which authorizes appointment of a legal guardian for adults with disabilities who cannot manage their own affairs. The process begins with obtaining a detailed medical certificate from a government hospital or civil surgeon confirming the diagnosis and extent of disability, preferably supported by a Disability Certificate issued under the Rights of Persons with Disabilities Act, 2016. Once these documents are ready, you must fill out Form-A available on the website or at the District Collector’s office, as the Collector is the Chairperson of the LLC.
