Gap affidavit/gap certificate to continue studies after 12th grade

Where can I get gap certificate from,will the lawyer provide me with gap certificate and I would only have to mention my reason of 1 year drop or would I have to write self declaration by myself and only get it stamped by lawyer.What can be the fees for gap certificate to lawyer that I would be paying.What are the documents I would have to carry to lawyer for gap certificate to continue further studies.How would I know if I am being provided with correct and valid gap certificate by the lawyer.
