correction of date of birth according to MUNCIPAL BIRTH CIRTIFICATE

MY ORIGINAL DOB IS 26 APRIL 1968 but my school certificate onwards recorded erroneously as 15june 1965 and my father name originally was R.V.RAMACHANDRA RAO but recorded as RMR Krsihna Rao in school certificate etc. advise how to process the rectification
