Changing of name in marriage certificate

My wife has my surname in our marriage certificate issued 6years back. We want to change it back to her maiden surname. All her documents (adhaar, passport, voter is, etc.) have her maiden name and we have not changed her name in any document after marriage. Please advise how to change her surname to maiden surname in marriage certificate.

as per your query you want to change the surname to maiden surname you need to apply for an affidavit from 1st class magistrate after that need to apply for gazette publication in regional language and English language paper
