Re. transfer of property after death and nomination

I have bought a flat and registered it in the name of myself and my wife. However in income tax return I claim my share as 100%. Currently the share certificate from society has my and my wife's name on it. I added my wife's name only as a convenience. I have following questions
1. On my passing away, will the ownership of the flat automatically pass on to my wife?
2. if we want to record my son as nominee do we have to jointly sign the nomination form?
