What is the procedure to remove my husband's name from passport?

I want to remove my husband's name from my passport. We are not divorced and i don't want to give a divorce notice as well. What is the procedure?
Without a pending divorce proceeding if you want to obtain a passport without the spouse name, you can can submit annexure K an Affidavit for Re-issue of passport, which was obtained prior to marriage, by married applicants who are unable to provide the prescribed marriage certificate or joint affidavit with spouse due to marital discord, separation or without formal divorce decree by the Court or due to total desertion by the spouse.
