Being harassed and followed by ex-boyfriend. What to do against him?

I am being harassed by my ex boyfriend since past one year. He follows me everywhere, gives blank calls on landline, keeps roaming around my house. I am getting married next month and he made a fake id on facebook with my pic and sent it to my fiance. What action can i take against that guy?
You can give a complaint in the local police station.based on the complaint police will take action accordingly and problem will be resolved. If the problem still persists then you can give a complaint to the DCP AND After that you can file the complaint before the court and court will take action and issue necessary directions accordingly.
