Under Section 13(3) of the Registration of Births and Deaths Act, 1969, deaths can be registered after one year only with magistrate’s order and supporting documents. Visit your municipality and Submit the following: Hospital death certificate (from Rajkot) Affidavit stating the death, cause, and delay (notarized) Identity/address proof of applicant (you) Witness statements (if any available from people who knew him in Gujarat) Supporting documents: Voter list, bank passbook, LIC policy, matric admit card, etc. Pay nominal penalty (if required) The Registrar will forward the case to the SDM or Magistrate, who can pass an order to issue the official death certificate.
