Same Name Affidavit and No Migration Certificate

My fathers name Bhokna son of late Sukur as per aadhar and Pan card. We residing in present permanent address village South Duillya, PO -Dullya, Dist -Howrah. My father originally belong to Jharkhand state . He came to West Bengal after getting permanent railway job in the year 1973.My father have some other name recorded in various documents in West Bengal such as Bhukan Karmakar son of sukur karmakar in voter ID card , Bhuban Karmakar son of shukul Karmakar in land deed year 1988 . Our paternal relatives use the surname Kerketta in Jharkhand state and they belong to the scheduled tribe. We have a relative caste certificate photocopy of Jharkhand state. Please advise for the same name affidavit for my father from honourable first class magistrates. Thank you.
