Regarding Changing my Name and DOB in Documents

I want to change my name from "Chander Parkash" to "Kartik Purohit". Because I don't like my current name. Along with I want to correct my DOB (There is a minor change in it, Only 22 February to 22 October). I want to know the process for it. Please Guide me.

For changing your name a publication in paper and a notification in gazette is required when your changed name appears in the gazette list with the help of the list after the formality you can get the changes done in your other documents.

there are three steps to do so. first and foremost you are required to get a publcation for the process to begin. then on your adhar card and PAN card it requires to be changed based on the publication. once the two steps are done then you can apply to the board to get your name and DOB changed. You may require the SDM order depending on where is your current residence
