Name correction in death certificate

My father died on 2001.. my mom lost the manual d.c given by that time.. later i checked on chennai corporation website,my fathers d.c is available with correct name, date and place( esi hospital).. but unfortunately grandfathers name was fully wrong.. there is someother name.. which office i have to approach for this issue?.. esi hospital?.. or corporation office of my zone?.. or gazette ofice? .. i only have voter id and school certificates of my father as id proof..

S.Gunasekar Advocate Answer. Approach the Chennai Corporation Zonal Office (where the death was registered) to apply for correction in the death certificate. Bring your father's school certificate, voter ID, and any other supporting documents. ESI Hospital cannot make corrections, and Gazette office is not needed for this.

Legal Process to Correct the Death Certificate: 1. Approach the Zonal Vital Records Department Go to the Chennai Corporation Zonal Office where the death was registered (based on ESI Hospital location) – NOT the ESI Hospital or Gazette Office. 2. Submit a Correction Application Request a “Death Certificate Correction” form. In the form, mention: Correct grandfather’s name Mistaken entry in existing certificate Justification and documents enclosed 3. Supporting Documents Required: Photocopy of the current (wrong) death certificate Father’s ID proofs showing correct lineage (Voter ID, School Certificate) Affidavit from Notary Public affirming correct grandfather’s name Optional: Legal heir certificate / Family ration card mentioning both names 4.
