Issue regarding gap year affidavit

A student who left the college within around 4 months and did not fill examination form or pay examination fees or attempted any external examination but also did not go through official withdrawal process is required to mention this in the gap year affidavit which is being submitted to another college after 1 year of dropping out of such a college ?
in this case you can prepare a detailed affidavit of gap or affidavit of Undertaking , explaining the gap caused due to any reason , be very cautious that you get it drafted from somebody as there need to be a detailed timeline explaining the gap , so that there may not be any further questions asked in regards to the same at the time employment or admissions
