Transfer of ownership of a property jointly owned by me and my father

I and my father jointly own a 3 bedroom apartment. Can I transfer all my rights or alternatively gift my share to him . Basically I want to relinquish all my rights on that property . My father and I live in different states. My health condition is not good so its very difficult for me. to travel back home. Please confirm will travel be mandatory and what are the formalities required to affect this transfer?
You will have to execute a relinquishment deed in the favour of your father thereby relinquishing your share in the property to him. Your presence is mandatory but if you cannot be present then you can execute a power of attorney in favour of someone who can then execute the relinquishment on your behalf
