Where can I apply for EPFO in which state?

My father used to work in a firm in Saharanpur (UP). He passed away in 2010. My mother is also no more. I have a younger brother aged 16 (minor). I contacted Employee Provident Fund Organisation (EPFO) for father's PF. I was advised to submit death certificate of both parents, other PF related forms and a Guardianship certificate. I don't have any information about Guardianship certificate. My question is our family belong to Uttarakhand. Father's job was in UP and I stay in Delhi as working in Noida. How and where to apply for it, in which of these 3 states? Thanks in advance.

First you need to check whether you are nominee named in the PF of your parents. If not then your application needs to be supported by a legal heir certificate. The application has to be made in Form 20 as provided in the Employees PF Scheme 1952. This application has to be submitted through the last employer of your father and mother. For instance, if your father was last working in UP, you need to submit this application through his last employer at UP who will then forward it to the concerned regional branch. Similarly, you need to follow the same procedure for your mother. If you cannot trace the last employer, then the application can be forwarded by the branch manager of the Bank where the PF account is maintained. The application needs to be supported by certain other documents such death certificate and guardianship certificate. For both a legal heir certificate and a guardianship certificate you have to apply to local courts where your parents were normally resident or where their death certificate was issued.
