Dear Client,
Since you have left your job with immediate effect due to your health issue, company in such circumstances may claim notice pay.

You may however demand the same by a letter followed by an official mail as an remainder and follow-up, addressed to your appointing authority, MD, Head-HR, then the company should supply you the following.
1) Acknowledgment and acceptance of your resignation.
2) Settlement of your a/c and dues payable to you.
3) Full and final statement.
4) Pf accumulation reports/withdrawal and transfer forms.
5) Work experience certificate,
6) NOC/NDC etc by registered/speed post so as to reach you in next say 7 days, (i.e. within 30 days from date of your resignation).

It is advised If you feel you may mention that you are enclosing self addressed postage prepaid envelope with your letter.

If the company does not respond you can issue reminder and if nothing works, you can approach to the Labor Commissioner/wages inspector.

Regards,
Tushar Bahadur,
(Advocate),
High Court Lucknow.
