Surname addition with the first name

My niece has all her documents with her first name only. She is applying for higher studies and scholarship in some universities in India and abroad, but Surname is a mandatory field. I want to understand the process of adding her surname to her documents and the possibilities.
In India a person can legally change his /her name unless it for notorious or malafide purposes. 1st step - Affidavit of change of name duly motorised. 2nd Step Notice of change of name in 2 newspaper 3rd Publication in Official Gazette. For legal services you may consult me.
