Name correction of my daughter

My daughter is 11 yrs old. Her name in birth certificate is Ishita and in school it is Ishita Sharma. I want to change her name to Ishita R Sharma. May pls advise.
please contact birth certificate branch in your city. that is situated in municipal office if you are from urban area. correction in birth certificate can be done. but you can add name but can not delete name once entered .
in simple name of your daughter in birth certificate would be like as ishita@ishita sharma after correction
you can not delete old name and enter new name in birth certificate you can only add another name which will be shown with old name . in case of your daughter birth certificate it will be shown as Ishita@Ishita Sharma
