The attestation form of government job asks to mention whether you have added or dropped any part of your surname or name.. now is she divorced. Is it compulsory to mention the previous husband's surname, which is added in the marriage and divorce certificate?

Yes. However, if you have not changed your name officially, then it would be necessary to drop husband’s name or add your maiden name through the prescribed method.
But anyways, if you are only filling the form, the certificate you mentioned would work.
