How to apply for the form of jaypee group

I owned a flat in Aman project from Jaypee Infratech and already paid 90% amount. I have taken a bank loan of Rs 34.14 lacs and in this amount I have utilized Rs 32 lacs and paying a EMI of Rs 32444 approximate. According to the circular of Insolvency and Bankruptcy Board of India I have to lodge my claim but not able to find the forms. In addition to the above I am also not able to understand that in which Form I have to file my claim i.e. FormB/C/D.
