You can file an FIR with the nearest Police Station and its respective Cyber Cell under the following penal provisions:

Section 354D, Indian Penal Code, 1860: This section deals with stalking, which includes the act of following, contacting, or attempting to contact a person repeatedly despite their clear indication of disinterest.

Section 354C, Indian Penal Code, 1860: This section deals with voyeurism, which includes the case where the victim consents to the capture of the image, but disagrees to the dissemination of the same, then the person who disseminates the image without the victim's consent can be held liable under this section.

Section 503, Indian Penal Code, 1860: This section deals with criminal intimidation, which is when someone threatens another person with harm to themselves, their reputation, or their property in order to force them to do something they wouldn't normally do or to prevent them from doing something they have a legal right to do.

Section 500, Indian Penal Code, 1860: This section deals with defamation, which is when someone intentionally makes a false statement or publishes an imputation about someone else with the intention of harming their reputation, or with the knowledge or belief that such imputation will harm their reputation.
For the creation of your fake social-media account - -
Section 66D of the IT Act, 2000: This section deals with cheating by personation, which is committed when a person fraudulently or dishonestly uses another person's electronic identity or email address, or any other electronic communication device or impersonates someone else to deceive or cheat someone. For instance, sending emails, and messages or making phone calls pretending to be someone else in order to obtain sensitive information, money or property. It is punishable by imprisonment of up to three years and a fine.

Section 66C of the IT Act, 2000: This section deals with the offence of identity theft and provides for punishment of up to 3 years imprisonment and/or a fine of up to Rs. 1 lakh for anyone who dishonestly or fraudulently makes use of the electronic signature, password or any other unique identification feature of any other person.
It is important to note that all major social media platforms such as Facebook, Instagram, Snapchat, etc. provide an option for users to report profiles that are fake or participating in unlawful activities. This system does not assure that all reported profiles will be removed, however, investigations are launched to cross-check the authenticity of reported profiles.
This will comes under IT act 21of 2000.
You can file a complaint against him under section 67 and 67A. Rest of the offence he has done under IPC as per your comments like threatening to upload, or to do something harmful to you and your family. Kindly proceed accordingly.
