Change of religion in respect of pension

My father has been drawing defence pension since late 70s ,now he is around 80 years old, off late we came to know that he is a Hindu, but we as his children were born and bought up in Muslim community, my mom was also a Muslim. Now for live certificate we have to approach to the bank in the month of November, some documents were found that he had converted to Islam during 80s,but did not inform the concerned department. Can you please let me know if it is possible now to do the procedure.

well your father can correct the situation by swearing an affidavit stating that he had converted to Islam on such and such date out of true will and faith upon the Islam religion. And his name was Mr. X before conversation and later he came to be known as Mr.Y, so Mr. X and Mr.Y is the same person having single identity.
Than you can submitt the affidavit to the bank with an application formally. The bank might demand some other documentary proof which you can submit and process the matter.
