Parents living separately mother wants to transfer guardianship

Parents are living separately after dispute from past 6 months, infant baby is with mother,father have not called or came to see the baby yet,now the mother wants to transfer guardianship to grandparents (maternal) what is the procedure..?

Maternal grand parents will have to file a guardian and wards O.P. in the District Court in the place where mother and baby resides showing mother as opposite party.
