Appoint a legal guardian ship of a mental I'll woman and her daughter

I have been maintaining a family for the last 6 years in that family a mental I'll woman and her 13 years old girl and father of a woman who is 80 years old . The woman father is now quit old who wants to be appoint me to legal guardian of her mental I'll daughter and her granddaughter. My question is it possible and if possible what to do
its basically a sort of an adoption process on your part but a 13 year old girl will only be handed over to you after full detailing and you have to guarantee of the upbringing and no misuse happens in regard to the female child and her security is also well taken care off
